Awards Day will be on May 16th at 10:00 AM, after Baccalaureate practice in the OPAC. Complete this form for each scholarship you have been awarded.  We will only present awards for those scholarships you have accepted.

Students will only be recognized at Awards Day if this form is submitted with all information. Indicate that you have accepted the award and will be present at Awards Day.  To be recognized, submit this form by Tuesday, April 22nd.  Should you receive additional scholarships/awards after this date, you may email Mrs. Belcher: beverly.belcher@opelikaschools.org, or Mrs. Hudmon: gayle.hudmon@opelikaschools.org
